Per the FDA Information Sheet – Frequently Asked Questions – Statement of Investigator (Form FDA 1572), currently approved sites, as well as sites pending approval, do not need to update their 1572s. If your site was approved after our move date of May 8, 2024, and your site had already submitted the 1572 with our old address or forgot to update our address, you do not need to send the FDA a revised 1572.
